AFL

The Alternative Family Living (AFL) Direct Support Professional (DSP) provides residential support to an individual with intellectual and/or developmental disabilities within the DSP’s primary residence. 


The AFL DSP provides a safe, stable, supportive, and caring home environment while assisting the individual in developing daily living skills, increasing independence, achieving individualized goals, and participating meaningfully in the community. Services are individualized according to each person’s needs, preferences, abilities, and service plan. The AFL DSP works in coordination with Integrity Providers, LLC and other members of the individual’s support team to promote health, safety, personal choice, dignity, and overall quality of life.


QUALIFICATIONS

- Must be 18 years of age or older.

- A High School Diploma or GED and an NC driver’s license with a safe driving record are required.

- Employment is conditional on background verification and passing a drug screen. 

- Related experience and education in Human Services is preferred.


This is a tax exempt position.

Office Admin

Provides administrative and clerical support to the Lead Admin and other designated team members at Integrity Providers, LLC. This position assists with recruitment, new-hire processing, interview scheduling, onboarding, employee documentation, records management, event coordination, public-facing communications, and general office administration.


The General Admin helps ensure administrative functions are completed accurately, timely, and professionally while maintaining the confidentiality of employee, member, and organizational information.


This is a W2 Employee Position.

Transportation Driver

Responsible for providing safe, reliable, and timely transportation for Integrity Providers, LLC members to and from approved destinations. Transportation may include travel to day programs, medical or other scheduled appointments, community activities, coordinated outings, job interviews, employment-related activities, and other authorized destinations.


The Driver is responsible for maintaining member safety and dignity throughout transportation, adhering to assigned schedules and transportation plans, communicating delays or concerns to appropriate staff, and complying with agency policies, confidentiality requirements, traffic laws, and applicable safety standards. The Transortation Driver reports directly to the Transportation Manager.


REQUIRES A VALID NC DRIVER'S LICENSE.


This is a 1099 Independent Contractor Position.

Transportation Manager

Provides direct care and support to clients in accordance with agency policies, procedures, and individualized service plans. Assists with daily living activities, health and personal care needs, and ongoing support for clients and their families to promote continued participation in the adult day program. Administers or assists with medication dispensing, monitoring, and reminders as permitted, and ensures medications are properly secured and stored according to program guidelines.


EDUCATION

- Must have at least a high school diploma or G.E.D certificate.

- Must have medication 103 management certification & training.

- Must have medication management certification/CPR & First Aid training.


QUALIFICATIONS

- Must be listed under the Nurse Aide I Registry.

- Must not have any prior convictions on the background check that includes:

a. Felonies related to manufacture, distribution, prescription or dispensing of controlled substance

b. Felony health care fraud

c. Felony for abuse, neglect, assault, battery, criminal sexual conduct {l st, 2 nd, or 3 rd

degree), fraud or theft against a minor or vulnerable adult

d. Felony or misdemeanor client abuse

e. Felony or misdemeanor involving cruelty or torture

f. Misdemeanor healthcare fraud

g. Misdemeanor for abuse, neglect, or exploitation listed with the NC Health Care Registry

h.  Any substantiated allegation listed with the NC Health Care Registry that would prohibit an individual from working in the healthcare field in NC.

i. Must not have any substantiated findings in accordance with the health care personnel registry.


EXPERIENCE

At least one year of health care experience is recommended for employees;

however, there is not a requirement for experience.


This is a W2 Employee Position.

DP/Para-Profesional

Provides direct care and support to clients in accordance with agency policies, procedures, and individualized service plans. Assists with daily living activities, health and personal care needs, and ongoing support for clients and their families to promote continued participation in the adult day program. Administers or assists with medication dispensing, monitoring, and reminders as permitted, and ensures medications are properly secured and stored according to program guidelines.


This is a 1099 Independent Contractor Position.

Qualified Professional/Case Manager

Responsible for assessing client needs, developing and maintaining individualized service plans, overseeing client records, and supervising paraprofessional/direct care staff to ensure services are delivered in accordance with agency policies, payer requirements, and applicable regulations.


The QP supports client care coordination, regulatory compliance, quality assurance, staff competency, and appropriate referrals while safeguarding client confidentiality, agency property, and intellectual property.


REQUIREMENTS

a. an individual who holds a license, provisional license, certificate, registration, or permit issued by the governing board regulating a human service profession, except a registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing who also has four years of full-time accumulated experience in MH/DD/SAS with the population served; or


b. a graduate of a college or university with a master's degree in a human service field and has one year of full-time, post-graduate degree accumulated MH/DD/SAS experience with the population served, or a substance abuse professional who has one year of full-time, postgraduate degree accumulated supervised experience in alcoholism and drug abuse counseling; or


c. a graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, post-bachelor's degree accumulated MH/DD/SAS experience with the 22-population served, or a substance abuse professional who has two years of full-time, post bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling;

or


d. a graduate of a college or university with a bachelor's degree in a field other than human

services and has four years of full-time, post-bachelor's degree accumulated MH/DD/SAS

experience with the population served.
