We are dedicated to providing efficient and effective services that will enhance the quality of life of our members. We provide services to men, women, and children that have mental illness, Substance Abuse, and HIV/AIDS diagnoses.

Our goal is to provide services that will allow our members to become functional and independent across all aspects of life including home, school, work, and community.

OUR PROMISES

  1. We promise to provide intensive and long term support to initiate and maintain major behavioral changes as it relates to our member’s mental health status.
  2. We promise to spend time developing trust and open communication with all our members regardless of their affliction, challenge or illness. 
  3. We promise to provide comprehensive case management services that adhere to the standards of CARF and the LME as our member’s health status fluctuates along the wellness/illness continuum.
  4. We promise to maintain our member’s confidentiality while providing a positive and strong presence in the community.
  5. We promise to employ the person centered process in the development of care plans.
  6. We promise to allow the client freedom of choice in the delivery of care and service coordination.
